UW Superior Women's Basketball Camps
Required Forms

All campers are required to fill out a camp Medical Consent and Release form. These forms must be received by our office prior to your camp participation.  You may either return the forms by fax to the Athletic Office at 395-4639 or bring to registration. These forms must be on file prior to camp in order for participation to be allowed.  If you are registering online, the required forms will be attached to an email you will receive immediately after registration.  If you would like you may also print out the required forms by clicking the link below.
